Office Policies

New Patient Appointment

We ask that you complete, sign and submit the new patient forms that you will receive by email or text prior to your first dental appointment. Alternatively, you may arrive 15 minutes before your first appointment to complete the simple new patient intake forms and review your medical history. Current radiographs are required in order to provide a baseline and to aid in your dental diagnosis and treatment plan. Current radiographs can be transferred from your previous office by signing a records release form in our office, signing a digital copy or by calling your previous office directly and requesting the transfer.

Appointment Rescheduling/Cancellation Policy

We schedule a convenient and specific appointment time for your dental care. If you cannot make your scheduled appointment, please call and give us 24 hours notice to easily re-schedule. We work to schedule efficiently and provide great care and we expect an honest commitment from you. A $50 administration fee will be charged due to missed appointments or short notice cancellations. Dental treatment can be unpredictable and time of treatment can sometimes exceed our expected timeline. We do our best to stay on time but we appreciate your understanding as we are focused on providing high quality treatment to every patient at every appointment.

Fees and Payment

We follow the Ontario Dental Association fee guide. We are committed to transparent fees. We can provide an estimate at your request. We maintain integrity in our billing practices. We are a fee-for-service clinic. Payment for services completed are due the day of your appointment, regardless of your insurance coverage. We do not accept assignment of insurance benefits. We will complete and submit your insurance claims for each appointment on your behalf and any reimbursement from your insurance company will be issued directly to you. At your request, we can send a pre-determination on your behalf to your insurance company and help you understand your coverage.

Canadian Dental Health Plan (CDHP)

We are currently accepting the CDHP. Please inform us immediately if you are using this program and provide your card. You can request to read our short booklet with information on the program to have a better understanding of the program. It is your responsibility to understand the program and to renew your card to maintain active coverage. We are accepting the program but the patient is responsible for the co-payments and balance-billing to meet the standard fee. Fees are collected the day of your appointment. The program is changing continually. We will assist you in navigating the program but ultimately you are responsible for all fees associated with your treatment.
